Wound Care Program

 
Affinity Home Health Care's Wound Care Program is designed to treat the full spectrum of skin care needs from minor skin irritations and injuries to complex wounds. Affinity Home Health Care's wound care nurses will assess the skin condition immediately,  will address the cause of the problem, report the patient’s condition to the physician and make a recommendation for treatment. The nursing staff will education patient and caregivers on prevention and management of wounds. Our nurses under go an extensive wound orientation and education program including the latest product advances and industry best practices including Wound VAC certification. Affinity Home Health Care's has had great success in treating non-healing and problematic wound care of patients at home in a comfortable and cost effective environment.

Our clinicians work in conjunction with the physician and local wound care center and continually evaluate wound healing and response to treatment. Preventing infection or other complications and helping the patient heal and return to their normal activities of daily living are main goals of the program. Patient education is a major component of our Wound Program. Education is tailored to the patient’s individual needs and focuses on eliminating factors that cause skin problems or interfere with healing.

Education on preventing infection, relieving pressure, controlling diabetes and the dietary nutrients needed for healing are included in the individualized care plan.
